Senior Care Manager -Caregiver Services Program

Provide information, assistance, prepare an appropriate caregiver support plan and coordinate services that will meet the existing needs for family caregivers.

Position Qualifications:

  • Master’s Degree in Social Work (or related Master’s level degree)
  • Minimum one year experience in social service organization Experience working with caregivers and/or older adults (preferred)
  • Fluent in a second language (preferred)

Responsibilities:

  • Conduct detailed intakes, initial in-home comprehensive assessments and re-assessment of family caregiver, including closing cases as appropriate
  • Develop caregiver support plans based on presenting needs including but not limited to respite and supplemental services) and coordinate services that will meet the needs
  • Caseload expected to include more complex, high-need, high-risk cases
  • With program supervisor/director, authorize services as appropriate and in keeping with eligibility requirements
  • Maintain up-to-date knowledge of and working relationships with community resources
  • Establish service linkages, make necessary referrals and monitor service delivery to successfully implement support plans
  • Provide assistance to caregivers on obtaining access to available services and resources in the community.
  • Provide advocacy services as appropriate
  • Maintain up-to-date case records on all caregivers, including completion of necessary statistical documentation
  • Maintain familiarity with all relevant NYC Aging standards, including updates to standards and procedures
  • Assist with other program activities as assigned
  • Additional responsibilities as assigned

Supervision and Education:

  • Supervised by Caregiver Services Program Director
  • Participate in team meetings
  • Participate in NYSOFA, NYC Aging, and CASC training
  • Participate in local educational and informational sessions and conferences
  • Annual personnel review
