POSITION SUMMARY: CRED is a cohort-based vocational training program for at-risk individuals between the ages of 18-40 years old. The program provides classroom instruction, case management services, internship opportunities, and job placement services designed to help clients earn a Commercial Driver’s License and obtain employment in the transportation industry. The Case Manager will be responsible for providing comprehensive case management services including, but not limited to academic and credentialing supports, career counseling, personal guidance and development, life skills, referral services, and family advocacy in a safe, caring, and enriching environment. The Case Manager will work in both individual and group settings to help CRED clients obtain employment, vocational credentials, and/or higher education. The Case Manager will support the coordination and execution of follow-up activities and attempt to retain all clients in their caseload. The individual selected for this position will be under the direct supervision of the CRED Program Director, will conduct themselves in a professional manner, and will comply with all standards set forth by United Activities Unlimited and the NYC Department of Youth & Community Development.

PRIMARY JOB RESPONSIBILITIES (include but are not limited to):

  • Conduct intake interviews to understand each client’s individual needs, strengths, weaknesses, and goals.
  • Complete Individual Service Strategies (ISS) for all CRED clients.
  • Maintain a caseload of approximately 60 clients, including check-ins and referrals to address barriers to employment
  • Provide individualized guidance to clients in their pursuit of college and/or career opportunities, including close collaboration with the Job Developer, Education Specialist, and relevant community partners.
  • Coordinate with the NYS DMV and CDL training vendors to schedule client’s training and certification appointments.
  • Complete and submit case notes for each participant, per DYCD standards.
  • Coordinate with Director and Education Specialist to prepare cohort schedules, including training, administration/proctoring, and validation of credentials to be achieved through program activities.
  • Attend weekly full-staff case management meetings to support regular sharing of up-to-date, relevant information on participants, and their progress and ensure ISS plans are adhered to or revised as necessary/appropriate.
  • Support the Director and Job Developer by maintaining positive working relationships
  • with social services agencies, community-based organizations, and other partners to enrich programming and employment opportunities.
  • Coordinate with community partners for referrals and other specialized services to deliver comprehensive wrap-around services that address client’s needs.
  • Advocate for and coordinate access to services, resources, and opportunities on behalf of clients, including childcare, housing, justice involvement, substance abuse, violence prevention, and/or access to public assistance benefits.
  • Collect, maintain, and audit participant files for completion and accuracy in accordance with DYCD and UAU standards.
  • Develop and maintain strong working relationships with staff, clients, and program stakeholders.
  • Participate in team and community partner meetings and events, and professional developments.
  • Assist with any other departmental duties and projects as needed.

JOB QUALIFICATIONS:

  • Associate’s degree required; Bachelor's Degree preferred
  • 2+ years of experience working in social services
  • Strong verbal and written communication skills
  • Excellent organizational and time management skills
  • Demonstrated ability to conduct and complete work independently
