Direct Support Professional (DSP) - Job Summary

• Assist program participants in carrying out their day-to-day life activities & goals according to the individualized treatment plans.
• Maintain a personalized, home like environment within the residence facility.
• Provide each individual the opportunity to be independent & productive according to their individual capabilities with opportunities for choice, physical and social integration into their community.
• Must be able transport individuals to & from medical appointments.
• Excellent organizational skills. High volume of filing. Documenting all medical appointments, follow ups are complete & diagnostic reports are obtained.
• Assist individuals to know, understand & exercise their personal rights as appropriate.

Direct Support Professional (DSP) - Responsibilities and Duties

• Provide training and hands-on intervention to individuals in the areas of personal care, self care, recreational services, communication, behavioral intervention and all other habilitation services offered to individuals as determined by their individual treatment plans and the cooperative efforts of other staff members.
• Assist in planning, coordinating, implementing and evaluating individuals’ service goals and total group home functioning and goals.
• Maintain accurate, up to date and complete records and reports.
• Transport individuals to social and recreational activities, medical appointments, and other destinations as required.
• Comply with all agency standards, rules, practices, policies, procedures and medication administration.

Qualifications and Skills

Valid N.Y. State Drivers License REQUIRED

Background working with individuals with intellectual disabilities preferred.

NO Highschool Diploma or GED Required

Salary

• $18.50 per hour
