The Home Health Aide is responsible for providing “hands on” care or direct care to the Assisted Living Program clients within the boundary of the DOH approved training for a Certified Home Health Aide. Plus, an accurate record keeping of any duties provided as directed by the Registered Nurse Supervisor, ALP Nurse and or Shift Supervisor.

The Home Health Aide is responsible for documentation (“sign off”) of the residents Plan of Care.

Minimum Requirements of the Job:

- At least 21 years of age

- High School Diploma or equivalent.

- Hold a current New York State Certificate for Home Health Aide

- Has experience in home care program

- Demonstrated caring attitude toward those in need of care

- Demonstrated ability to motivate self and work in a team

- Able to read, write and communicate clearly in English Language.

- Familiar with the needs of the infirm and elderly.

- Be able to complete work in time allotted.

- Be Well Organized.

GENERAL TASKS

Every attempt has been made to outline all of the details of this position. However, there may be other tasks inherent in the position that may not be listed below.

1. Provide assistance to clients according to the care plan developed by the nursing supervisor in accordance with all tasks trained to be performed as listed in the DOH Memorandum 92-24 “Home Health Aide Scope of Tasks” published on August 19, 1992. Such tasks include the following the directions of the ALP Nurse/LPN/HHA Supervisor/Shift Supervisor.

2. Assist with nutritional support: Assist in measuring, recording, monitoring and supervising intake.

3. Assist with bathing residents: tub, shower, and/or sponge bath.

4. Assist with changing linen: change/assist with bed linen for the incontinent residents.

5. Assist with grooming: Shampoo, nail care, shave, oral hygiene, denture care, dressing, undressing, straightening clothing, applying elastic stockings.

6. Assist with toileting: assisting with urinal, toilets, apply, remove diaper/pull-ups, apply condom catheter, change/empty leg bag catheter, and assist with measuring out put and record.

7. Assist with walking: assist with use of durable medical assistive device (wheelchair, rollator, cane and walker) inside or outside the facility.

8. Assist with skin care: keep clean, apply lotion, gentle massage, lubrication and powder. Application of Topical Creams, lotion and ointment.

9. Assist with transferring, positioning and range of motion: transfer from bed, chair, toilet bowl or wheelchair using proper body mechanics, sliding board or Hoyer lift.

10. Assist with medical supplies or equipment, (such as walkers, wheelchairs, etc.), assist with prosthesis, artificial limbs, or appliances, hearing aids, artificial eyes, etc.

11. Assist with changing a clean dressing of a stable wound, remove old dressing, cleanse skin around wound, and apply new dressing.

12. Assist in measuring and recording residents’ weights. ANY DRASTIC CHANGE MUST BE REPORTED IMMEDIATELY TO SUPERVISOR OR NURSE.

13. Completing floor and meal census. Always know the general where about of the Residents. Familiarize yourself with all the Residents.

14. Review Residents Plan of Care on a weekly basis for any change in the care of the residents. Ensure all Aide Activities Sheets are filled out appropriately, and any changes necessary reported to Nurse or HHA Supervisor.

15. Monitoring room temperatures and smoking in residents room every two hours. Also monitor residents who drink alcohol in the facility.

16. Assist residents with any room changes, assist in obtaining a new key. Secure residents belongings. Record room change on floor and meal census sheets.

17. Accompanying residents on outside appointments as well as in house.

18. Assisting with laundry service. Document laundry dropped off and picked up (please check laundry with residents before signing sheet: YOU WILL BE HELD ACCOUNTALBE IF NOT DONE PROPERLY.)

19. Assist in maintaining a safe, clean and comfortable environment. Reporting repairs to front desk and the need to clean assigned areas, to supervisor. Always report findings to supervisor.

20. Observing and reporting any change in the residents’ status. The need for higher level of care, referral for further specialized care and any inappropriate behavior.

21. Participate in on the job training and in-services as planned by the Facility.

22. Any other tasks that may be directed by the ALP Nurse, HHA Supervisor or Shift Supervisor.
