JOB PURPOSE AND OBJECTIVES

Provide case management services to adults age 60 and older in order to identify client’s needs and service preferences, and work with client (and family when appropriate) to develop, implement and oversee a care plan to meet mutually agrees upon goals.

 

ESSENTIAL FUNCTIONS:

  • Conduct assessments for PEC clients.
  • Develop individualized care plans based on client needs and preferences, and in collaboration with client’s family member(s) and CASC team members.
  • With program supervisor, authorize EISEP homecare, housekeeping and home delivered meals as appropriate, based on client eligibility guidelines.
  • Assist with intakes.
  • Maintain up-to-date-knowledge of, and working relationships with community resources.
  • Establish service linkages, make necessary referrals and monitor service delivery to successfully implement individualized care plans.
  • Provide benefits and entitlements counseling and assistance applying for and follow-up to ensure receipt of benefits entitlements.
  • Provide advocacy services for and on behalf of clients.
  • Maintain up-to-date case records on all clients, including completion of necessary statistical documentation.
  • Maintain familiarity with all relevant Department of the Aging standards, including updates to standards and procedures.
  • Assist with PEC calls as assigned.
  • Assist with other program activities as assigned.

 

SUPERVISORY RESPONSIBILITIES:

None

 

 

EDUCATION, EXPERIENCE AND SKILLS:

  • BSW, BS, BA level education is required.
  • Experience in providing direct services to the elderly preferred.
  • Preferred fluency in a second language.
  • Word, Excel essential, and Department of the Aging database VIVE (preferred).
  • Detail oriented, organized, understands diversity, self-motivated, and a creative problem solver.
  • Experience working with older adults (preferred).
  • Self-disciplined: manages own time, meets deadlines, works well under pressure
  • Attention to details.
  • Analytical: reviews options, develops systematic and logical procedures, and present clear and logical solutions.

  • Case Management clients receive high quality, individualized services and supports to help them remain safely in their homes and communities.

SUPERVISION AND EDUCATION:

  • Supervisory reviews conducted by consultant.
  • Participate in team meetings as needed.
  • Participate in CASC training and education sessions as needed.
  • Performance reviews.
  • Bachelor’s Degree
