Program Assistant FVP

Case Management Program for Older Adults

Purpose and Objectives: Assist Friendly Visiting Program Coordinator with managing program needs, administrative tasks and outreach; support volunteers and clients to engage in a mutually rewarding, meaningful relationship that will contribute to a reduction in client social isolation.

We do this by working in hand-in-hand with older adults, respecting their right to make decisions for themselves, honoring their life experiences and heritage, and recognizing the importance of friends, relatives and community in creating supportive environments for people as they age.

Position Qualifications:

  • Bachelor’s level degree and one-year experience in social services experience; or
  • High school diploma or an AA degree and two years social service experience;or
  • Four years of direct social service experience. 

 Requisite Skills and Capabilities:

  • Computer skills; Excel knowledge

  Responsibilities:

  • Assist Friendly Visiting Program Coordinator with recruiting, screening, interviewing, placement and monitoring volunteers
  • Effectively set up tracking systems to manage volunteer contact information, recruitment resources, areas of interest, skills, and other information to place volunteers accordingly
  • Creating program forms and outreach materials as directed by Friendly Visiting Program Coordinator.
  • Facilitating community outreach, presentations to clubs, organizations, groups, fairs, local markets and businesses to recruit and cultivate volunteers
  • Assist Friendly Visiting Program Coordinator coordinate the match between the Client/Older Adult and Volunteer and follow through to ensure the match is going well and is successful
  • Responsible for record keeping of referrals
  • Effectively record and maintain case notes and VIVE information
  • Coach, train and orient FV volunteers as directed by Friendly Visiting Coordinator
  • Participate in Supervision, Staff Meetings, Staff Development, NYC Aging and CASC trainings and meetings when necessary
  • Monitor and maintain program productivity expectations as determined by NYC Aging as directed by Friendly Visiting Program Coordinator
  • Perform other duties and special projects as assigned
  • Other Administrative and Supervisory tasks as assigned by Friendly Visiting Program Coordinator

 Key Results:

  • Help reduce social isolation and related feelings of sadness experienced by homebound older adults.

 Supervision:

  • Supervised by Friendly Visiting Program Coordinator
