Assistant Director of Case Management

Position Qualifications:

  • MSW/MS required
  • Experience in providing direct services to the senior population preferred
  • 2+ years supervisory/administrative experience
  • SIFI certified, or ability to obtain certification preferred

 

Requisite Skills and Capabilities:

  • Strong Computer Skills: fluency in Microsoft Word, Excel, Outlook required, Knowledge of NYC Aging database VIVE is a plus
  • Assessment Skills: ability to thoroughly assess needs of older adults inquiring about services
  • Organized: ability to manage time, job responsibilities and prioritize tasks appropriately
  • Leadership: ability to identify strengths and weaknesses in staff and provide supervision, education, training and guidance to improve job performance, and to manage staff during crisis and other stressful situations
  • Self-motivated: able to work independently, work well under pressure and handle multiple priorities simultaneously
  • Understand diversity: accommodate linguistic and cultural differences; work well with colleagues and clients with diverse ethnic and cultural backgrounds and can guide staff in meeting these goals
  • Creative problem-solving during crisis situations and complex client presentations

Responsibilities:

  • Assist the Program Director in quality improvement including running of reports to monitor caseloads, deadlines, productivity, quality of services and other data
  • Conduct supervisory reviews as part of quality assurance
  • Assign clients in STARS to designated Case Managers in consultation with Program Director, including assessment of prioritization for assignment
  • Manage and improve workflow; develop and implement systems to support staff in managing workload
  • Design and implement training tools for current and new team members
  • Identify training opportunities for Case Management Program
  • Ensure that all NYC Aging procedures and requirements are fulfilled, including maintaining NYC Aging required caseloads
  • Oversight of STARS data management system, ensuring timely and accurate data input
  • Supervise Intake Coordinator in oversight of the Case Management Intake Department and supervision of Case Aide; including conducting and reviewing intakes and monitoring quality of intakes conducted

·         As directed by PD, oversee and monitor client service reports and provide as needed support in coordinating supplemental supplies

  • Supervise social work interns, including weekly supervision and oversight of documentation
  • Supervise Case Managers during program supervisor vacancy or as directed by Program Director; may include supervision of small CM team as program grows
  • Escort Program Supervisors and/or Case Managers on high risk/high needs home visits as needed
  • Support Program Director in hiring, orienting new hires and conducting new hire trainings
  • Support Program Supervisors as directed by Program Director, including providing clinical support for high risk/high needs clients
  • Participate in staff meetings, supervisory meetings and NYC Aging and CASC trainings, including co-facilitating meetings as directed by Program Director
  • Collaborate with outside service providers to best address client needs and staff training needs
  • Maintain familiarity with all relevant NYC Aging standards including updates to standards and procedures
  • Provide program coverage in Program Director’s absence
  • Evening or weekend supervisory coverage during weather/emergencies
  • Other administrative and supervisory tasks as assigned by Program Director

 

Supervision and Education:

  • Directly Supervised by Program Director
  • Supervise Intake Coordinator, Case Aide when Intake Coordinator is absent, Student Interns
  • Temporary supervision of CM teams, during Supervisor vacancy as directed by PD
  • Participate in team meetings
  • Participate in NYC Aging and CASC training and education sessions
  • Participate in local educational and informational sessions and conferences
  • Annual personnel review
  • Assigns cases in consultation with Program Director

  • Case Management Program operates effectively and efficiently
  • Case Management clients receive high quality, individualized services and support to help them remain safely in their homes and communities
  • Effective implementation of CASC procedures and guidelines

 

Salary and Benefits:
