Job Description:

The Open Doors Transition Center is funded by the NYS Department of Health as part of the federal Money Follow's the Person, (MFP) Demonstration, which assures that residents of long-term care facilities are provided objective information and given the opportunity to live in a community based setting.

Qualifications:

  • Minimum of an Associates Degree in Social Services, a related field or equivalent experience.
  • Experience with the concept of Independent Living, (IL), consumer control, and a strong commitment to promote and advocate for the full integration of individuals with disabilities.
  • Experience transitioning individuals from nursing homes back in the community preferred.
  • Knowledge of state Medicaid Program and community resources that assist people with disabilities in living independently.
  • Experience with Nursing Home Transition and Diversion (NHTD), Traumatic Brain Injury (TBI), and OPWDD waivers as well as Medicaid Managed Long Term Care (MLTC) strongly preferred.
  • Experience with NYC social services agencies (e.g; HRA) strongly preferred.
  • Excellent interpersonal, organizational, computer, and communication skills.
  • Frequent travel to nursing homes in Staten Island and outer boroughs if needed.

Responsibilities:

  • Work collaboratively with NYAIL's Open Doors team to provide transition services with a focus on Staten Island.
  • Participate in the yearly NYAIL conference in Albany, NY, regional trainings, and one-on one Transition Specialist trainings.
  • Utilize program database to identify individuals, and regional training needs.
  • Meet regularly with Open Doors staff to ensure the accuracy of lists and follow-up on requested items.
  • Must travel to nursing homes in Staten Island, and other boroughs if needed.
  • Work collaboratively with NYAIL's Peer Outreach and OHS programs.
  • Data entry and Consumer Service Records on CIL Database.
  • Perform other duties as assigned by SICIL Management.
